Job SummaryWe are seeking a highly skilled Clinical Pharmacist to join our multidisciplinary team. As a key member of our healthcare team, you will contribute to the overall desired patient outcomes through provision of pharmaceutical care, including drug therapy monitoring, pharmacokinetic consultation, drug information, and other activities related to drug use control.
Key Responsibilities- Assess, plan, implement, and evaluate drug therapy effectively, ensuring the best possible outcomes for patients.
- Educate patients about medications to empower them with knowledge and promote autonomy, reducing non-compliance and complications.
- Apply the pharmaceutical care model to identify, resolve, and prevent drug-related problems, promoting a culture of safety and quality.
- Conduct drug utilization reviews to optimize medication use and minimize costs.
- Coordinate the acquisition and supply of in-center and home medications, ensuring seamless continuity of care.
- Participate in formulating the interdisciplinary plan of care, ensuring comprehensive and coordinated treatment.
- Provide effective supervision, leadership, and direction to Pharmacy Technicians, students, and volunteers, fostering a supportive and inclusive work environment.
- Bachelor's Degree in Pharmacy from an accredited institution.
- Licensed with the Ontario College of Pharmacists in Part A of the Register (or eligible for registration).
- Previous hospital experience required, demonstrating proficiency in clinical pharmacy practice.
- Hospital residency or equivalent experience.
- PharmD degree or certification in pharmaceutical care.
- Demonstrated expertise in pharmaceutical care, communication, and computer skills.
