Job Description
The Mental Health Care Coordinator will work collaboratively with interprofessional care teams to provide comprehensive treatment and care management services for individuals with mental health and/or concurrent disorders. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Key Responsibilities
- Coordinates care for clients by working with interprofessional care teams to develop and implement individualized care plans.
- Provides direct nursing care to clients in compliance with standards of practice, including reviewing client/family history, conducting physical/psychosocial assessments, and collaborating with the healthcare team to implement treatment modalities.
- Works from a strengths-based model, using motivational interviewing and engagement techniques to develop and implement individualized care plans.
- Establishes and enhances therapeutic relationships with clients and their support network by engaging stakeholders in treatment planning and evaluation.
- Provides treatment services, including crisis intervention and individual counseling, according to various theoretical models.
Qualifications
- Graduation from an approved School of Nursing or Psychiatric Nursing.
- Two (2) years recent related experience providing treatment services to individuals with mental health and/or concurrent disorders.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
