We are seeking a highly motivated and detail-oriented individual to join our team as a Case Support Specialist. In this role, you will provide critical support to Investigations and Focus teams, working closely with primary social workers to develop and implement case plans that meet the needs of children and families.
Key Responsibilities- Review and analyze current case plans, identifying areas for expansion and improvement.
- Conduct research and gather information from collateral sources to inform case decisions.
- Accompany social workers on interviews with children, parents, caregivers, and other relevant parties.
- Conduct home visits to observe family dynamics and living conditions, providing education and support as needed.
- Arrange and attend medical exams for children, providing information to healthcare professionals.
- Prepare and maintain accurate documentation, including case notes and service plans.
- Coordinate with social workers to arrange placements and gather necessary information.
- Compile and gather information to make referrals to community agencies as required.
- Provide regular updates to social workers and supervisors.
- Transport clients as needed, ensuring safe and timely arrival at appointments.
- Engage in case planning activities to prevent separation and preserve family integrity.
- Make referrals to appropriate services as assessed by social workers.
- Maintain ongoing communication with service providers and community partners.
- Develop partnerships with service providers and community organizations to enhance service delivery.
