Key Responsibilities:- Conduct comprehensive assessments of patients' functional abilities and develop personalized treatment plans.
- Provide occupational therapy services to patients in their homes and communities, promoting independence and safety.
- Collaborate with healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care delivery.
- Document patient progress and treatment plans in accordance with regulatory requirements.
- Participate in multidisciplinary teams to develop and implement care plans that meet patients' needs.
- Registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario.
- Experience working with diverse patient populations, including those with complex needs.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work independently and as part of a team, with a focus on patient-centered care.
- Valid driver's license and access to a vehicle.
