We are seeking a highly skilled and compassionate Cultural Health Navigator to join our team at DIVERSEcity Community Resources Society. As a Cultural Health Navigator, you will play a vital role in improving health outcomes for government-assisted refugee populations who experience language and cultural barriers navigating the healthcare system.
Key Responsibilities- Work with refugee populations to identify immediate health needs and bridge primary healthcare access by providing 1-to-1 health orientation, navigation, and promotion in a culturally safe, trauma-informed way.
- Support overall health promotion of clients through providing information on health and health behaviors, and health programs/resources available.
- Deliver clear, timely, and appropriate pathways through service bridging and referrals for healthcare access within cultural context to remove barriers.
- Accompany clients to medical appointments and work collaboratively with healthcare staff to provide cultural bridging between clients and healthcare practitioners.
- Empower independent healthcare system navigation by orienting clients on the Canadian healthcare system and newcomer services.
- Collaborate with healthcare staff on healthcare planning for patients in developing individualized and effective service plans.
- Carry out continuous service delivery monitoring, evaluation, and quality assurance to provide data on client needs and process quality indicators.
- Degree in Health Sciences or related field and two years related experience.
- Minimum 2-3 years experience in settlement case management.
- Fluency in reading, writing, and speaking English and Rohingya/Burmese (Rohingya preferred).
- Demonstrated understanding of social determinants of health and how they impact racialized refugee populations.
- Knowledge of the government-assisted refugee experience and settlement challenges.
- Knowledge of IFHP and MSP insurance coverage.
- Extensive knowledge of health systems and refugee-focused programs in the Lower Mainland.
- Demonstrated ability for active listening, building trust, and advocating for client needs and resources.
- Demonstrated understanding of cultural and traditional health practices and beliefs of the communities being served.
- Demonstrated ability to work independently, take initiative, solve problems, and work as part of a team.
- Working knowledge of standard office equipment and software applications (Microsoft 365 and CRM).
