Nurse Practitioner

Vancouver Coastal Health is seeking a skilled Nurse Practitioner to join our team in providing high-quality care to vulnerable home-bound and frail patients. As a key member of our interdisciplinary team, you will be responsible for providing semi-longitudinal care, working closely with patients, families, and healthcare professionals to ensure comprehensive and coordinated care.
Key Responsibilities- Conduct comprehensive assessments of patients and clients, including diagnosing diseases, disorders, and conditions.
- Develop and implement individualized care plans, including health care management, therapeutic interventions, and medication management.
- Collaborate with patients, families, and healthcare professionals to identify and address health trends and patterns.
- Participate in quality improvement activities and contribute to the development of policies and procedures that promote best practices in patient care.
- Provide education and support to patients and families to promote health and well-being.
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
- Current registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM).
- 4 years of recent, relevant clinical nursing experience.
- Successful completion of the Objective Structured Clinical Examination (OSCE).
