Job Summary:
  • Provide social work services to elderly clients, including assessment, counseling, and case management.
  • Develop and implement individualized care plans to meet the unique needs of each client.
  • Collaborate with healthcare professionals, family members, and community agencies to ensure comprehensive care.
  • Provide education and support to clients and their families on aging-related issues, such as dementia, chronic illness, and end-of-life care.
  • Conduct home visits and assessments to identify client needs and develop strategies for support.
  • Work with clients to access community resources, such as home care, adult day programs, and respite care.
  • Participate in interdisciplinary team meetings to discuss client care and develop plans for support.
  • Document client interactions, progress, and outcomes in a timely and accurate manner.
Requirements:
  • Master's degree in Social Work (MSW) or a related field.
  • Experience working with elderly populations, preferably in a geriatric social work setting.
  • Strong assessment and intervention skills, with the ability to work with clients of diverse backgrounds and needs.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong commitment to client-centered care.
  • Valid driver's license and reliable transportation.
