Career Opportunity

We are currently seeking a Senior Community Care Nurse to join our team in Cornwall & Stormont County. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to our clients, promoting independence, and supporting them in achieving their optimal level of well-being.

Job Summary

The successful candidate will possess excellent clinical skills, a strong knowledge base, and excellent communication and interpersonal skills. You will be responsible for assessing client needs, developing and implementing personalized care plans, and collaborating with other healthcare professionals to ensure seamless care delivery.

Key Responsibilities
  • Conduct thorough assessments of clients' physical and emotional needs
  • Develop and implement comprehensive care plans that address client goals and objectives
  • Provide skilled nursing care, including wound management, medication administration, and education
  • Collaborate with multidisciplinary teams to ensure coordinated care delivery
  • Communicate effectively with clients, families, and healthcare professionals
Requirements

To be considered for this role, you must have:

  • A current registration with the provincial licensing body (RPN/LPN)
  • Valid driver's license and reliable vehicle
  • Excellent clinical skills and knowledge base
  • Strong communication and interpersonal skills
