Support and Education Coordinator, First Link, Chinese Community Engagement

The Support and Education Coordinator, First Link, Chinese Community Engagement position plays a vital role in delivering the Society's First Link programs and services within an assigned region. This role involves working independently and as part of a regional and provincial team to connect clients to support services and information, and delivering standardized workshops. Some travel within the region is required, with a hybrid work-from-home option available 2-3 times a week.
Key Responsibilities- Under the guidance of the Manager, Community Services, ensure the Society's core First Link services are consistently delivered at the community level, utilizing various delivery methods and responding to the needs of the primary target groups.
- Respond to calls on the First Link Dementia Helpline (Chinese) as part of the Chinese Service Focus Team to provide supportive listening, assess caller needs, recommend information and resources, make referrals to other Society dementia support programs, and complete intakes as needed.
- Develop and maintain relationships with healthcare professionals through presentations, informal and formal contacts, and information exchanges to build a referral network.
- Maintain proactive outreach and ongoing follow-up with people with dementia and their families, and connections to services and supports.
- Lead collaboration with Resource Development and Marketing and Communications on fundraising and awareness activities for First Link for Chinese Communities, including event planning, marketing, and media outreach, and overseeing translation activities.
- As needed, support and facilitate delivery of standardized workshops for caregivers, people with early symptoms of dementia, and the public.
- As needed, support delivery of support groups for family/friend caregivers and people with early symptoms of dementia.
- Support Minds in Motion Contractors in their work.
- Maintain an effective volunteer base to help deliver programs and services, as needed.
- Maintain a client and referral sources database, ensuring recordkeeping is complete, objective, and current.
- Represent the Society at the community level, building relationships with other service organizations, local businesses, politicians, and municipalities in alignment with department goals and the Strategic Plan.
You have a post-secondary education or relevant degree in health or social agency-related functions or an equivalent combination of education and experience. You bring related support and education experience in a human services environment, including work such as public speaking, presenting structured education programs, group facilitation, peer support interventions, volunteer management, and developing and building communication connections.
Key Qualifications- Fluency in Cantonese required, Mandarin an asset.
- Extensive knowledge of and experience with Alzheimer's disease/dementia and caregiving issues.
- Strong organizational and time management skills, able to work independently, strong attention to detail.
- Flexibility regarding scheduling is expected - some evening and weekend work may be required.
- Must have access to reliable transportation.
