This role requires a deep understanding of community care principles and the ability to work independently in a fast-paced environment.

Your Responsibilities:

  • Complete comprehensive client assessments to identify their unique needs and develop personalized care plans
  • Implement and evaluate care based on best evidence and practices, working collaboratively with other care team members
  • Support clients through care transitions by connecting them with community support services and resources

Requirements:

  • Current registration with the College of Nurses of Ontario
  • CAN specialist certification an asset
  • Enthusiasm for community care and a passion for delivering exceptional patient care
