Job Summary
Mackenzie Health is seeking a highly skilled Social Worker to join our Emergency Department team. As a Social Worker, you will play a vital role in providing psychosocial assessment and support to patients and their families, ensuring they receive optimal care throughout their hospital stay.
Key Responsibilities
- Conduct thorough psychosocial assessments to identify patients' needs and develop effective care plans.
- Provide counseling, financial planning assistance, and patient/family education to promote optimal psychosocial functioning.
- Collaborate with the interprofessional team to ensure seamless care coordination and discharge planning.
- Advocate for patients and families, addressing concerns and facilitating access to community resources.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
- Current registration with the Ontario College of Social Workers and Social Services Workers.
- Master's degree in Social Work from an accredited University.
- Minimum of three (3) years of previous Social Work experience in a hospital environment.
- Minimum of 2-3 years current related clinical experience in a Critical Care setting.
Preferred Qualifications
- Advanced interpersonal, coaching, and leadership skills.
- Excellent decision-making, problem recognition, and problem-solving skills.
- Proven attendance record.
