We are seeking a highly skilled Registered Nurse to join our team as a Patient Care Manager. As a key member of our healthcare team, you will be responsible for providing high-quality patient care, working collaboratively with other healthcare professionals to achieve the best possible outcomes.

The successful candidate will have strong leadership and management skills, with the ability to work effectively in a fast-paced environment. They will be knowledgeable about nursing science and practice, with a commitment to ongoing professional development and continuing education.

In this role, you will:

  • Develop and implement individualized care plans, working collaboratively with interdisciplinary teams.
  • Communicate effectively with patients, families, and colleagues, using clear and concise language.
  • Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care.
  • Coordinate care with CCAC when patients are discharged, ensuring that their care is adequately maintained.

To be successful in this role, you will need:

  • A current registration with the College of Nurses of Ontario.
  • Current BCLS/HCP certification.
  • Demonstrated knowledge, skill, and expertise in Best Practice Guidelines pertaining to vascular access across a variety of patient populations.
  • Demonstrated experience in Peripherally Inserted Catheter (PICC) insertion technique.
