Job Summary

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Baycrest. As a Geriatric Care Specialist, you will provide high-quality, patient-centered care to our residents, working collaboratively with our interdisciplinary team to develop and implement individualized care plans.

Key Responsibilities
  • Provide Primary Healthcare and Follow-Up Care: Provide comprehensive, collaborative healthcare to a designated group of residents, including assessment, diagnosis, and treatment of diseases, disorders, and conditions.
  • Communicate Effectively: Communicate findings, diagnoses, and health outcomes to residents, families, and the interprofessional care team, ensuring seamless care coordination.
  • Lead and Collaborate: Assume a leadership role within the program and interdisciplinary team, developing solutions and service delivery models that promote exceptional resident and family care.
  • Advocate for Residents: Provide leadership in advocating for residents at the point of care, across the organization, and at the health system/policy level.
  • Drive Clinical Inquiry: Encourage and model critical clinical inquiry, driving the development of nursing research and improving resident care.
  • Identify and Address System Needs: Identify needs and make recommendations for changes within the system to enhance both clinical practice and resident outcomes.
Qualifications
  • Nurse Practitioner Certification: Possess a Nurse Practitioner-Primary Health Care ('NP-PHC') or Nurse Practitioner-Adult ('NP-Adult') certificate from a recognized program.
  • Gerontology Nursing Certification: National certification in Gerontology Nursing from the Canadian Association of Nurses (CAN) is preferred.
  • Geriatric Care Experience: Minimum two (2) years providing geriatric care in one of the following settings: long-term care, community, homecare, and virtual care.
  • Leadership and Interpersonal Skills: Demonstrated effective leadership skills, including flexibility, creativity, and commitment, as well as excellent interpersonal skills, including conflict resolution, mentoring, and coaching.
  • Research and Program Planning: Demonstrated research knowledge, program planning, implementation, and evaluation skills.
