Key Responsibilities:

  • Connect clients with community resources and services

  • Collaborate with healthcare professionals to ensure comprehensive care
  • Maintain accurate records and documentation

Requirements:

  • Certificate or diploma in social work, counseling, or related field
  • Minimum 2 years experience working in mental health and addiction services
  • Proven ability to work with diverse populations
  • Valid driver's license required
