About the Role
We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Home and Community Care Support Services. As a Nurse Practitioner, you will play a vital role in providing high-quality, patient-centered care to our clients in their own homes.
Key Responsibilities
- Provide holistic patient care based on advanced nursing and basic medical management knowledge and skills for a selected patient population.
- Liaise with patients, families, and all healthcare providers to ensure seamless care coordination.
- Conduct medication reconciliation and reviews medication protocol with patients and caregivers.
- Identify patients requiring an accelerated assessment and home care services, and work with the Care Coordinator to facilitate the home assessment visit.
- Participate in establishing, maintaining, and monitoring standards for Case Management, including committee work and quality initiatives.
Requirements
- Membership, in good standing, with the College of Nurses of Ontario.
- Registered Nurse in the Extended Class (RN EC designation).
- Working knowledge of community resources and roles of healthcare professionals.
- Specialized education in palliative pain and symptom management.
- Palliative/End of Life Care experience.

- Pay Rate: Wages are in accordance with the collective agreement between Ontario Health atHome and OPSEU and salaries range from $54.377/hr. - $63.161/hr.
