Job Summary

We are seeking a highly skilled and compassionate Home Healthcare Occupational Therapist to join our team in Scarborough Region. As a key member of our community access occupational therapy team, you will be responsible for providing timely and effective occupational therapy interventions to clients with complex needs.

Key Responsibilities
  • Assess and implement occupational therapy interventions in a community setting, adhering to professional practice guidelines and standards established by VHA and the College of Occupational Therapists of Ontario.
  • Conduct comprehensive assessments of clients using interviews, standardized and informal assessment techniques, and develop personalized treatment plans to address their unique needs and goals.
  • Provide evidence-based interventions to promote independence, mobility, and overall well-being, including but not limited to: home safety equipment and mobility aids, falls prevention strategies, energy conservation techniques, palliative care, ADL training, cognitive remediation, and linking to community resources and funding.
  • Communicate effectively with clients, families, and healthcare teams to ensure coordinated delivery of care and optimal outcomes.
  • Maintain accurate and detailed records of assessment findings, treatment plans, progress, and discharge summaries, as required by the profession, referral sources, and VHA.
Requirements
  • Degree in Occupational Therapy from a recognized school of Occupational Therapy.
  • Member in good standing with the College of Occupational Therapists of Ontario.
  • Experience within the professional discipline is an asset.
  • Specialized courses to support areas of clinical expertise.
  • Ability to work independently and effectively communicate with diverse stakeholders.
  • Familiarity with valid and reliable outcome measures to support clinical interventions.
  • Demonstrated commitment to client- and family-centered care.
  • Ability to recognize safety concerns and communicate effectively using technology.
