We are seeking an experienced registered physiotherapist to join our team at Ontario Health atHome Champlain. As a physiotherapist, you will play a vital role in providing connected, accessible, patient-centered care to our patients in the community.

Key Responsibilities:

  • Assess and determine patient care needs and eligibility
  • Provide access and referrals to community services
  • Engage with patients, caregivers, and other healthcare practitioners
  • Plan, organize, and direct diverse programs of treatment, counseling, screening, and education

Requirements:

  • Degree or diploma in physiotherapy and current certificate of registration with the College of Physiotherapists of Ontario
  • Related professional experience in a physiotherapy role
  • Fluency in English and French is required
  • Vulnerable Sector Check completed within the last six months
