Key Responsibilities:
- Assess and determine patient care needs and eligibility
- Provide access and referrals to community services
- Engage with patients, caregivers, and other healthcare practitioners
- Plan, organize, and direct diverse programs of treatment, counseling, screening, and education
Requirements:
- Degree or diploma in physiotherapy and current certificate of registration with the College of Physiotherapists of Ontario
- Related professional experience in a physiotherapy role
- Fluency in English and French is required
- Vulnerable Sector Check completed within the last six months
