Job Summary
We are seeking a highly skilled and dedicated Nurse Practitioner to join our team as a Community Health Specialist. This is a unique opportunity to work in a dynamic and challenging environment, providing comprehensive health services to individuals and families in our communities.

Job Description
As a Nurse Practitioner - Community Health Specialist, you will be responsible for providing primary and specialty care services to patients in their homes and in our clinics. Your duties will include conducting patient assessments, developing care plans, and providing education and support to patients and their families. You will also be required to work collaboratively with other healthcare professionals to ensure seamless care coordination.

Required Skills and Qualifications
To be successful in this role, you will need to have:

  • A bachelor's degree in nursing and a master's degree in nursing or a related field;
  • At least two years of experience as a nurse practitioner or one year of experience in an emergency room setting;
  • Current registration with the O.I.I.Q and a valid nursing license;
  • Fluency in English, with Cree or French being an asset;
  • Strong communication and interpersonal skills;
  • Ability to work independently and as part of a multidisciplinary team.

Salary
The estimated annual salary for this position is $85,000 - $115,000, depending on experience.

Other
This is a full-time position, with a regular schedule and occasional on-call periods. You will be required to participate in ongoing education and training to maintain your certification and stay current with best practices.
