The Southlake@Home Program Coordinator will play a pivotal role in managing the day-to-day operations of our innovative care program. As a key member of our interdisciplinary care team, you will work closely with patients, caregivers, and healthcare providers to deliver high-quality, patient-centered care in the comfort of patients' own homes.

Key responsibilities include:

  • Coordinating care activities, including scheduling appointments and arranging transportation
  • Maintaining accurate records and tracking patient progress
  • Communicating effectively with patients, caregivers, and healthcare providers
  • Facilitating transitions to alternative care settings when necessary

A successful Program Coordinator will possess strong organizational and communication skills, as well as the ability to work collaboratively with diverse stakeholders.
