At Njoyn, we are committed to providing exceptional care to our community. As a Social Worker, you will be an integral part of our clinical care team, working closely with Geriatricians and Nurse Practitioners to provide direct support to complex seniors in an interprofessional outpatient clinic setting.
">">Key Responsibilities:
- Perform comprehensive chart reviews and preliminary assessments/case histories utilizing high-level clinical judgement and critical thinking skills.
- Act as a liaison between patients, consultants, resources, and referral sources, including crisis intervention between family members, hospitals, agency service providers, police, and patients.
- Provide patient and family-centered assessment and management of individual needs, including social dynamics, elder abuse, adjustment counseling, and illness understanding.
- Facilitate linkages with primary providers and community supports as needed.
- Document all relevant information and ongoing contacts with patients/families/professionals that impact patient needs or intervention by the care team.
- Participate in identifying issues and formulating/revising departmental policies and procedures relevant to clinical operations.
Requirements:
- Masters' Degree in Social Work from an accredited university required.
- Member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW), with no terms, conditions, or limitations required.
- Minimum 2 years of experience working with the geriatric population.
- MOCA certification required, or willing to obtain upon hiring.
