About the Role
CarePartners is seeking a highly skilled Occupational Therapy Assistant/Physiotherapy Assistant to join our interdisciplinary care team in the community/home setting. As a key member of our team, you will provide one-on-one care to patients, delivering therapy within the care plan and working independently or in cooperation with the Ontario Health atHome team.
Key Responsibilities
- Deliver professional treatment to patients in the community and in their homes
- Assist with gross motor, fine motor, and balance activities
- Work with patients to set and achieve time-specific, measurable goals and outcomes in collaboration with the care team
- Provide client/family education and support to address physical, functional, and self-care needs as assigned
- Monitor and report changes in health status to the supervising Occupational Therapist/Physiotherapist
Requirements
- Occupational Therapist Assistant/Physiotherapist Assistant diploma from an accredited Ontario college program
- Current First Aid Certificate and Cardiopulmonary Resuscitation (CPR) Certificate
- Demonstrated understanding and practice of rehabilitation principles and techniques
- Sound critical thinking skills, good judgment, and problem-solving skills
- Ability to participate and follow treatment programs and care plans
