Job Summary

The Government of Northwest Territories is seeking a highly skilled and experienced Home Care Registered Nurse to join our team. As a Home Care Registered Nurse, you will be responsible for providing high-quality nursing care to clients in their homes, working independently and collaboratively with other healthcare professionals to deliver comprehensive care.

Key Responsibilities
  • Provide advanced nursing care to clients in their homes, including wound care, palliative care, chronic care, and foot care.
  • Conduct home visits to assess client needs and develop individualized care plans.
  • Collaborate with other healthcare professionals, including physicians and community health workers, to ensure comprehensive care.
  • Manage client caseloads, prioritize tasks, and coordinate care with other healthcare providers.
  • Participate in quality improvement initiatives to enhance client care and outcomes.
Requirements
  • Current registration with the Registered Nursing Association of the Northwest Territories and Nunavut (RNANT/NU).
  • Minimum 2 years of recent, acute care nursing experience in a medical, surgical, home care, or community health environment.
  • Valid Class 5 Driver's License.
  • Ability to work independently and collaboratively in a fast-paced environment.
  • Excellent communication and interpersonal skills.
