About the Role

The Patient Care Specialist will be responsible for providing exceptional care to our residents, ensuring their physical, emotional, and social needs are met. This includes assisting with daily living activities, such as bathing, dressing, and grooming, as well as monitoring vital signs and reporting any changes to the healthcare team.

Duties and Responsibilities
  • Provide direct care to residents, including bathing, dressing, and grooming
  • Assist residents with daily living activities, such as feeding, toileting, and ambulation
  • Monitor vital signs and report any changes to the healthcare team
  • Deliver meals and snacks to residents
  • Transport residents between facilities
  • Participate in resident care conferences and contribute to the development of individualized care plans
Requirements

To be successful in this role, you will require:

  • A high school diploma or equivalent
  • Experience in a patient care setting, preferably in a long-term care facility
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
