About the Role:
We are seeking a skilled Community Support Specialist to join our team at ILNSA. As a Community Support Specialist, you will be responsible for providing support to clients in their homes and community settings.
Key Responsibilities:
- Prepare reports and case histories to document client progress
- Develop and implement service plans to meet client needs
- Liaise with other social services agencies and healthcare providers to ensure comprehensive support
- Assist clients with daily living tasks and provide emotional support
- Manage program statistics and evaluate the effectiveness of treatment programs
Requirements:
- Secondary school graduation certificate
- 1 year of experience in a related field
- Valid driver's license and reliable transportation
- Ability to work independently and as part of a team
