We are seeking an experienced Occupational Therapist to join our team in Surrey, B.C., as a member of the Assertive Community Treatment (ACT) team. The successful candidate will work closely with the interdisciplinary team to assess clients' functional capacity, occupational performance status, and environmental supports, developing individualized rehabilitation plans that address their unique needs. They will also provide direct individual and group intervention to support clients in achieving their goals and maintain regular contact with community-based agencies to advise the team on resources and facilitate effective entry and participation in rehabilitation services.
The responsibilities of this position include:
- Conducting comprehensive assessments of clients' functional capacity, occupational performance status, and environmental supports.
- Developing and implementing individualized rehabilitation plans that address clients' unique needs and goals.
- Providing direct individual and group intervention to support clients in achieving their goals.
- Consulting, referring, and linking clients to life skills, education, employment, and leisure programs.
- Maintaining regular contact with community-based agencies to advise the team on resources and facilitate effective entry and participation in rehabilitation services.
