Key Responsibilities- Conduct comprehensive assessments of patients' physical and cognitive functioning to identify areas of need and develop personalized treatment plans.
- Collaborate with patients, families, and healthcare professionals to implement treatment plans and achieve optimal outcomes.
- Develop and implement individualized treatment plans based on analysis and clinical reasoning, incorporating evidence-based practices and best practices in occupational therapy.
- Provide education and training to patients and families on topics such as safe body mechanics, cognitive behavioral strategies, and functional tolerance activities.
- Support return-to-work programs, including job coaching and accommodation recommendations, to help patients achieve their goals and regain independence.
- Utilize electronic charting software to document treatment sessions and submit reports to stakeholders in a timely manner.
- Participate in case conferences and case management to ensure seamless communication and coordination of care.
- Collaborate with the inter-professional team to provide comprehensive care and achieve optimal patient outcomes.
- Supervise occupational therapy students, as preferred, to support their professional development and growth.
- Network with stakeholders to promote pt Health's occupational therapy programs and services.
- Manage a caseload in a fast-paced environment, prioritizing tasks and managing time effectively to meet deadlines and achieve goals.
- Registration (member in good standing) with the provincial College of Occupational Therapists.
- Previous experience treating clients with physical, neurological, and/or mental health conditions.
- Ability to work autonomously and as part of a team, demonstrating strong communication and interpersonal skills.
- Demonstrated commitment to post-graduate professional development, staying current with best practices and evidence-based research in occupational therapy.
- Previous experience supporting auto insurance, long-term disability, and WSIB referrals is a strong asset.
- Experience supporting occupational rehabilitation is a strong asset.
- A valid driver's license and access to a reliable vehicle.
