Job Description

In the role of Licensed Practical Nurse at Optima Living, you will provide professional nursing care to residents in our Independent Living, Assisted Living, Supportive Living and Memory Care communities. As a key member of our team, you will work closely with residents, their families, and other healthcare professionals to deliver exceptional care.

Your Responsibilities:

* Develop comprehensive care plans that address the physical, psychological, and social needs of residents under the direction of the Director of Care or designate.
* Direct and assist care aides in providing high-quality care, ensuring adherence to established procedures and protocols.
* Administer, monitor, and record medication administration, providing education to residents as needed.
* Collaborate with interdisciplinary teams to create and manage nursing care plans, participating in evaluations of current and future medical needs.
* Evaluate the effectiveness of nursing care treatments and communicate significant changes in resident conditions to the Director of Care or designate and/or the resident's physician.
* Provide personal care assistance, nursing procedures, and mentorship and coaching to Health Care Aides.
* Work with residents to meet care requirements, communicating concerns to the doctor and manager as necessary.
* Demonstrate ongoing competence through utilization of knowledge, skill, judgment, and evidence-based practice in caring for aging adults.
* Ensure compliance with provincial continuing care health service standards, legislation, codes, regulations, and employment and safety statutes.

Qualifications and Experience:

* Completion of a Nursing diploma program.
* Registration with the provincial nursing regulatory body and good standing.
* Experience in a residential care setting is preferred.
* Ability to demonstrate infection control procedures.
* Exceptional interpersonal skills with peers, residents, visitors, and operational partners.
* Superior organizational skills with the ability to handle multi-resident requests.
* Superior oral and written communication skills.
* Demonstrated ability to prioritize and problem-solve.
* Flexibility to work a variety of shifts.
* Ability to stand for extended periods, bend, squat, kneel, and lift 25 pounds.
