We are seeking a highly skilled and compassionate Rehabilitation Case Manager to join our team at FunctionAbility Rehabilitation Services. As a key member of our multidisciplinary team, you will play a vital role in providing expert rehabilitation services to individuals who have suffered traumatic and acquired neurological injuries and severe orthopedic injuries.
Key Responsibilities- Coordinate rehabilitation services for clients, ensuring a seamless and effective treatment plan.
- Act as a liaison between clients, healthcare professionals, and other stakeholders to ensure consistent information sharing and follow-through on medical recommendations.
- Coordinate service referrals and community resources as required, including referrals to other healthcare professionals, transportation services, and pharmacy accounts.
- Complete paperwork on behalf of clients to seek reimbursement for medical and rehabilitation expenses.
- Provide detailed, comprehensive reports as part of the medical-legal system.
- Masters Degree in Social Work, Occupational Therapy, or Nursing.
- Current registration and membership in good standing with a regulated college.
- Minimum 2 years of experience in case management and/or family counseling.
- Strong verbal and written communication skills, with the ability to work with diverse personalities.
- Valid driver's license and access to a vehicle.
