We are seeking a highly skilled and experienced Clinical Services Partner to join our Transitional Care team at Shannex Incorporated. As a key member of our team, you will play a vital role in providing quality patient care and supporting the transition of patients across various healthcare settings.
Key Responsibilities- Collaborate with the Transitional Care Team to support and care for patients and their support networks during transitions within and across settings.
- Research and provide evidence-based recommendations for nurses and members of the Transitional Care Team to support safe and effective transitions in care.
- Work closely with Shannex and Nova Scotia Health to identify clinical education needs for employees and support the development of a clinical education plan for roll out.
- Deliver clinical and IPAC training sessions, workshops, and educational programs, and monitor their effectiveness.
- Participate in the development, evaluation, and revision of clinical policies, procedures, and education consistent with legislation, best practices, and Accreditation standards.
- Lead quality improvement initiatives and present data findings and quality improvement project outcomes as requested.
- Participate in Accreditation preparations and conduct regular clinical and IPAC audits to determine compliance with regulatory requirements, clinical policies, and procedures.
We are looking for a highly skilled and experienced Registered Nurse (RN) with a strong background in clinical leadership and a passion for quality patient care. You will have a Bachelor of Nursing or a Bachelor of Science in Nursing, and current registration or eligibility to be registered with the appropriate nursing regulatory body. A minimum of five years of nursing experience, including clinical, administrative, and leadership experience, is required.
Experience using clinical outcomes and data to evaluate and guide clinical performance activities is essential. A Master of Nursing, Master of Science in Nursing, or another healthcare-related post-graduate program is considered an asset. Completion of post-graduate clinical education and/or certifications, such as Restorative Care, Gerontology, Palliative and End-of-Life Care, Behavioural Supports, Skin & Wound Care, and Infection Prevention & Control (IPAC), is also an asset.
