We are seeking a dedicated Ambulatory Healthcare Specialist to join our team at Island Health. This role offers an exceptional opportunity for medical professionals to provide primary care services in a collaborative and innovative environment.
As an Ambulatory Healthcare Specialist, you will work closely with other healthcare providers to deliver comprehensive care to patients in their homes. Your expertise and commitment to preventative medicine will play a crucial role in ensuring that patients receive timely and effective treatment, reducing the need for hospital admissions.
This position is ideal for medical professionals who value flexibility, autonomy, and collaboration. You will have the opportunity to work on a Priority Response Team (PRT) alongside registered nurses and occupational therapists, providing urgent care and support services to patients in need.
The PRT operates 7 days a week, providing approximately 7 hours of coverage per day. As an Ambulatory Healthcare Specialist, you will be responsible for managing your shared schedule and working collaboratively with the team to ensure seamless patient care.
Your key responsibilities will include:
- Responding to emergent referrals or consults from primary care providers
- Assessing and treating housebound patients for acute conditions
- Screening and triaging patients referred for priority care
- Offering clinical assessments and developing treatment plans
- Diagnosing and treating acutely ill patients to prevent hospital admissions
You will also have the opportunity to participate in educational and training initiatives to enhance your skills and knowledge in ambulatory care.
In this role, you will enjoy a competitive salary of $250,000 - $300,000 per annum, commensurate with experience. Additionally, you may be eligible for rural incentives, including relocation costs reimbursement and a fee premium.
