Job Description:

We are seeking an experienced RN Case Manager to join our team in Valcartier, QC. As a RN Case Manager, you will be responsible for providing case management services to our clients who suffer from protracted and complex health issues.

Key Responsibilities:

  • Complete detailed case management assessments
  • Develop comprehensive care plans that reflect individual health care needs
  • Monitor clients and make suggestions for modifications as necessary

Requirements:

  • 3 years of experience in clinical nursing practice
  • Previous case management experience
  • Baccalaureate Degree in Nursing from a recognized university
  • Current license to practice from a provincial or territorial College of Nurses in the province of practice
