We seek a healthcare professional who possesses a strong foundation in nursing practices, techniques, and methods applied to health and wellness resident care. The ideal candidate will have a graduate degree in nursing, a current state license as a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN), and a minimum of one year experience in home health, assisted living, hospital, or long-term care environments.

Key Responsibilities

  • Promote the overall well-being of residents through ongoing needs assessments and service delivery
  • Participate in resident evaluations and data gathering to ensure high-quality resident services
  • Assist Resident Care Director in conducting resident evaluations and data gathering information
  • Conduct regular wellness visits for residents to identify potential health issues early on
  • Monitor and follow up with residents who have experienced falls, accidents, or incidents
