Vancouver Coastal Health is seeking a permanent Family Physician to join our team at HomeVIVE.
Job Summary:
HomeVIVE provides primary care house calls to frail seniors in Vancouver. As a Family Physician, you will be part of an interdisciplinary team providing care to a panel of patients.
Responsibilities:
- Manage patient care on a day-to-day basis, including chronic disease management and palliative care.
- Provide house calls, virtual visits, and manage medications.
- Correspond with allied health providers and be available for urgent issues.
- Participate in after-hours call group and provide timely house calls to clients when needed.
- Partner with Home and Community Health to promote team-based care and prevent unnecessary ER visits and admissions.
Qualifications:
- Licensure in good standing with the College of Physicians and Surgeons of British Columbia.
- Current Canadian Medical Protection Association coverage.
- Canadian College of Family Physicians (CCFP) designation.
- Interest in frail elder care and ability to work in a close-knit team environment.
