**Job Summary**
We invite you to join our interprofessional care team as a Mental Health Therapist. In this role, you will provide treatment and care management services to individuals with mental illness and concurrent disorders, promoting inclusionary and flexible approaches to meet the unique needs of each client.
Responsibilities:
- Collaborate with the interprofessional care team to coordinate care for clients, performing intake functions, conducting mental health assessments, and developing comprehensive treatment plans based on client needs and best practices.
- Plan and provide direct nursing care to clients in compliance with standards of practice.
- Work from a strengths-based model, using motivational interviewing and engagement techniques to promote positive outcomes.
- Establish, maintain, and enhance therapeutic relationships with clients and their informal support networks.
- Provide treatment services including crisis intervention, individual counseling, and education.
- Initiate and participate in case conferences/meetings with clients, family, service providers, and members of the interprofessional team.
- Consult and collaborate with members of the interprofessional care team.
- Monitor Extended Leave as required under the Mental Health Act.
- Aid client access to services such as medical appointments, hospitals, and social programs.
- Document assessments, care plan, observations, interventions, client/family responses, outcomes/evaluations, and referrals according to practice standards.
- Maintain a safe environment for clients, visitors, and employees.
- Identify own learning needs/goals.
- Participate in research and quality improvement activities.
- May be required to plan, coordinate, and deliver educational sessions.
- May be required to provide outreach nursing care to clients in their home setting.
- May include working with residential facilities or community agencies/homes.
- As assigned: demonstrates and articulates the responsibility of the authorized RN/RPN prescriber.
- Performs other related duties as required.
