About the Role
We are seeking dynamic and passionate Therapy Assistants (OTA/PTA) to join our Transitions team at SE Health, a not-for-profit social enterprise that brings hope and happiness to clients, homes, and communities across Canada.
Job Description
As a Therapy Assistant, you will be part of a diverse, collaborative care team providing services to clients in the community with a focus on holistic care and client-centered goals. You will work closely with clients and families to deliver care adhering to SE Health's policies and procedures, adopted from Ministry of Health Guidelines.
Responsibilities:
- Provide care to clients and families, using effective communication tools to address their physical, cognitive, social, emotional, and spiritual domains.
- Communicate changes in the client's condition to the primary care lead/clinician and other members of the care team as required.
- Work collaboratively with the care team to provide a comprehensive plan of care and achieve client-centered goals.
Requirements
To succeed in this role, you will need:
- A minimum of one year of relevant work experience in home/community care, acute care, chronic disease management, health promotion, or palliative care.
- Experience working in a community setting is an asset.
- Excellent communication skills, both written and verbal.
- The ability to exercise good judgment and work independently.
- A vehicle and valid driver's license for travel purposes.
