About the Role

We are seeking a highly motivated and compassionate Community Support Services Worker to join our team at Pacific Coast Community Resources.

Key Responsibilities
  • Provide care and support to individuals living in staffed homes within their community, promoting independence and self-determination.
  • Encourage and empower individuals to make informed decisions about their lives, respecting their rights and choices.
  • Develop and implement Personal Service Plans in collaboration with individuals, families, and multidisciplinary teams.
  • Foster positive relationships with individuals, families, professionals, and community members, ensuring effective communication and advocacy.
  • Record daily activity and personal health information on our case management database, maintaining accurate and up-to-date records.
  • Contribute to maintaining a clean and well-maintained living environment, including daily food preparation and light housekeeping duties.
Requirements
  • Community Support Worker Diploma or equivalent.
  • Minimum 2 years of experience in community social services or healthcare.
  • Current Emergency First Aid/CPR and Food Safe certificates.
  • Previous behavior management course/training.
  • Ability to effectively communicate verbally and in writing with team members, professionals, families, and community members.
  • Restricted Class 4 driver's license (for Full-Time positions).
  • Criminal record clearance for children and vulnerable adults.
  • Proof of COVID-19 vaccination status.
  • TB Test.
  • Moderate computer literacy.
