The Mental Health Social Worker is a key member of the Mental Health Team, responsible for providing exceptional patient care and support to individuals with serious mental illness.
Primary Responsibilities- Conduct comprehensive assessments and develop treatment plans for clients in the Adult Outpatient Mental Health program.
- Provide short-term counseling and psycho-social support to individuals and groups, utilizing evidence-based modalities such as Brief Solution Focused model, cognitive behavioral, and dialectical behavioral therapy.
- Document clinical assessments, treatment plans, and client progress, ensuring accurate and timely reporting to the team.
- Lead client groups, demonstrating expertise in group dynamics and facilitation skills.
- Liaise with team members, hospital staff, and community agencies to promote collaboration and good working relationships.
- Develop and deliver education programs to the community and hospital staff.
- Collect and analyze statistical data to inform program development and improvement.
- Participate in committee work and other duties as required.
- Masters in Social Work (MSW)
- Three to five years of experience working with seriously mentally ill clients
- Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Advanced counseling and assessment skills, including knowledge of various short-term counseling modalities
- Excellent communication, analytical, and problem-solving skills
- Ability to work collaboratively in an interprofessional team environment and independently
- Regular attendance and commitment to customer service and teamwork
- Nonviolent Crisis Intervention Course (CPI) or equivalent
This role requires a strong commitment to providing exceptional patient care and support, as well as a passion for working collaboratively with diverse stakeholders to promote mental health and well-being.
