We are seeking a skilled Nurse Coordinator to join our team at New Brunswick Extra-Mural (EMP)-Programme extra-mural (PEM) du Nouveau-Brunswick. As a key member of our healthcare team, you will be responsible for providing holistic care to patients, incorporating the nursing profession's values of knowledge, theory, and practice.

• Assess, Plan, Implement, and Evaluate patient needs, collaborating with interdisciplinary teams to deliver high-quality care.
• Coordinate Patient Care by working closely with healthcare professionals to ensure seamless delivery of services.
• Develop and Implement Care Plans tailored to individual patient needs, incorporating knowledge of community services and resources.
• Communicate Effectively with patients, families, and healthcare teams to ensure comprehensive care.
• Collaborate with Interdisciplinary Teams to achieve patient goals and improve health outcomes.
• Provide Education and Support to patients and families on health-related topics and disease management.
• Participate in Quality Improvement Initiatives to enhance patient care and services.

• Diploma in Nursing or Bachelor Degree in Nursing from an accredited institution.
• Active Registration with the Nurses Association of New Brunswick or ability to register.
• Minimum of Two Years of specialized nursing experience in family practice, surgical, palliative care, or oncology.
• Knowledge of Target Population and available community services.
• Valid New Brunswick Class 5 Driver's License and good driving record.
• Clean Criminal Record Check and current CPR Level C certification.
• Excellent Interpersonal and Leadership Skills, with ability to work efficiently as a team member.
