As a Community Health Navigator, you will be part of our dynamic team that supports vulnerable populations.

Your Key Responsibilities:

  • Work closely with an inter-professional team to provide comprehensive support services to clients
  • Collaborate with clients to identify and overcome barriers to their well-being
  • Promote client-centered practice and recovery orientation
  • Manage time effectively and work independently in client homes and community settings
  • Support clients in problem-solving, safety planning, harm reduction, and resource access
  • Offer counseling, referrals, and advocacy
  • Navigate healthcare systems for clients

Requirements:

  • Degree in social service/social work or relevant field
  • Minimum 3 years' experience in human or health services
  • Excellent problem-solving, risk management, and crisis intervention skills
  • Knowledge of local community service agencies and systems
  • Outreach/community-based work experience preferred
  • CURRENT ONSLAUGHT drivers license
