Overview
CarePartners is seeking a skilled and compassionate Registered Nurse or Registered Practical Nurse to join our team in a community nursing role. As a community nurse, you will have the opportunity to work in a variety of settings, including in-home, clinics, and retirement homes, providing exceptional nursing services to patients and their families.
Key Responsibilities
- Develop and maintain effective care plans in collaboration with the interdisciplinary care team.
- Provide nursing services to patients in their homes, clinics, and other community settings.
- Communicate and liaise with patients, family members, and other healthcare professionals to ensure seamless care.
- Perform other clinical duties as required.
Requirements
- Current registration with the College of Nurses of Ontario (CNO).
- Valid driver's license and vehicle.
- Excellent English verbal and written communication skills.
- CPR certification.
- Current immunization record, including 2-step TB test.
- Vulnerable sector check and clear background check.
