Job Summary
Vancouver Coastal Health is seeking a Social Worker to join the team at Lions Gate Hospital in North Vancouver, BC. As a Social Worker with Vancouver Coastal Health, you will be responsible for providing clinical and consultative social work services to clients and families.
Key Responsibilities
- Conduct social assessments and provide counseling services to clients and families.
- Maintain accurate records and participate in interdisciplinary clinical/case conferences.
- Identify and connect clients with community resources and facilitate care throughout the continuum.
Qualifications
Education and Experience
- Bachelor of Social Work from an accredited School of Social Work and one (1) year post-Bachelor of Social Work experience or successful completion of a practicum in a healthcare setting.
- Current full registration with the British Columbia College of Social Workers.
Knowledge, Skills, and Abilities
- Basic knowledge of systems thinking as it relates to social work process.
- Basic knowledge of counseling skills.
- Basic knowledge of other health disciplines and their role in healthcare.
- Basic knowledge of legislation and government policies and procedures that relate to clients.
- Demonstrated ability to conduct social assessments.
- Demonstrated ability to apply systems theory to practice in social work assessment and intervention.
- Demonstrated ability to communicate effectively with clients, families, team members, and community agency personnel.
- Demonstrated ability to work collaboratively as a member of an interdisciplinary team.
