About the Role

CarePartners is seeking a dedicated and compassionate Personal Support Worker to join our team in providing high-quality care to patients in their own homes. As a key member of our eHomeCare Program, you will work collaboratively with specially trained nurses to deliver innovative and patient-centered care.

Key Responsibilities
  • Deliver care and support to patients and their families in their home
  • Provide quality, integrated care through direct communication between a nurse via laptop/tablet and the eHomeCare Support Worker via smartphone throughout their shift
Requirements
  • A strong desire and commitment to making a difference in the lives of their patients
  • Passion for providing holistic, sensitive, individualized care to patients at end of life
  • Experience with providing care to patients in their own homes (considered an asset)
  • Ability to work independently and take direction and feedback effectively
  • Technological skills such as smartphones and virtual care delivery platforms
  • Exceptional interpersonal skills
  • Ability to work both independently and as part of a team
  • Availability to work non-traditional hours, such as 8-hour night shifts and alternate weekends
  • A valid driver's license and vehicle
  • Valid certificate as a Personal Support Worker or Internationally Trained Nurse or enrollment in a Nursing program
  • To protect our patients, we require current vulnerable sector check screening, and a clear background check
