As a Patient Care Coordinator, you will be responsible for coordinating patient care services, ensuring timely and efficient delivery of care. This includes communicating with patients, families, staff, and funder organizations to facilitate seamless care delivery.

Your primary responsibilities will include:

  • Staff Optimization: Utilize scheduling software to optimize staff schedules and meet their availability and client care needs.\
  • Record Keeping: Maintain accurate records in the EMR to support quality care delivery.\
  • Issue Resolution: Address patient concerns promptly and follow escalation pathways to resolve issues efficiently.\
  • Professional Development: Participate in training and professional development opportunities to enhance skills and knowledge.

In addition to your technical skills and experience, we value your strong ability to communicate effectively, both written and orally, in English. Your positive work and attendance performance, as well as your ability to work independently and prioritize workload, are also essential for success in this role.
