Clinical Social Worker, Hospice Care Team

We are seeking a skilled and compassionate Social Worker to join our Hospice Palliative Community Services team in the Tri Cities area. As a key member of our interdisciplinary care team, you will provide essential social work services to clients and their families, promoting their well-being and quality of life.
Key Responsibilities- Conduct comprehensive social and psychosocial assessments of clients and families, identifying their unique needs and circumstances.
- Develop and implement individualized care plans, working collaboratively with the interdisciplinary team to ensure seamless care coordination.
- Provide counseling and therapy services to clients and their families, addressing grief, bereavement, and other psychosocial issues.
- Liaise with community agencies, hospitals, and other healthcare providers to ensure comprehensive care and support.
- Participate in quality improvement initiatives, contributing to the development and evaluation of program goals and objectives.
- Collaborate with colleagues and community partners to develop and provide educational programs and resources on psychosocial issues related to illness and disability.
- Master's degree in Social Work from an approved School of Social Work.
- Two years' recent, related clinical experience working with palliative clients, or an equivalent combination of education, training, and experience.
- Current full registration with the BC College of Social Workers and a valid BC Driver's license.
- Knowledge and understanding of counseling skills and clinical practice models as it relates to individuals, families, groups, and palliative care.
- Ability to work effectively in a multicultural environment and as part of an interdisciplinary team.
- Strong communication and interpersonal skills, with the ability to develop and maintain rapport with clients, families, and colleagues.
