Primary Care Nurse Practitioner

We are seeking a highly skilled Nurse Practitioner to join our team at the Toronto Western Family Health Team, Garrison Creek. As a key member of our interprofessional team, you will provide high-quality patient-centered care to a diverse outpatient population.
Responsibilities:
- Provide full-scope primary care services using evidence-based practices and guidelines
- Collaborate with healthcare professionals to coordinate patient care and address complex health issues
- Develop and implement individualized care plans that prioritize patient needs and preferences
- Foster strong relationships with patients and their families to promote health education and empowerment
Requirements:
- Demonstrated expertise in primary care nursing and advanced practice skills
- Current certification as an RN(EC) or equivalent in the province of Ontario
- Master's degree in Nursing or related field; Primary Care Nurse Practitioner Certificate an asset
- 5+ years of experience in a primary care setting, with a minimum of 1 year in an advanced role
