Key Responsibilities:
- Provide comprehensive home health care services to clients, including wound care, IV therapy, and medication administration.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Conduct health teaching and education to clients and their families.
- Participate in ongoing professional development and training to maintain clinical excellence.
- Work independently and as part of a team to achieve client-centered care goals.
Requirements:
- Registered with the College of Nurses of Ontario in good standing.
- Palliative training is considered an asset.
- Pediatric experience is considered an asset.
- A clear criminal record check with vulnerable sector screening.
- Proof of negative TB test and professional liability insurance.
- Ability to travel within the community.
