About the Role

Mendip Vale Medical Group is seeking a skilled Social Prescriber Link Worker to join our team. As a Social Prescriber Link Worker, you will play a vital role in empowering patients to take control of their health and wellbeing by addressing the wider determinants of health.

Main Responsibilities

  • Provide personalized information, advice, and support to patients to meet their needs, circumstances, and preferences.
  • Develop trusted relationships with patients, clinicians, and community groups to connect them to community and statutory services.
  • Support the development of new community groups where there are gaps in services.

Requirements

  • NVQ Level 2 in Health or Social Care or other relevant professional academic qualification.
  • Understanding of the wider determinants of health, including social, economic, and environmental factors.
  • Excellent written and verbal communication skills.
