Home Admissions Specialist

We are seeking a highly organized and detail-oriented Home Intake Coordinator to join our team at TRC Total Renal Care Inc. As a key member of our administration support team, you will play a vital role in overseeing the admission process for new home patients and patients transitioning from in-center hemodialysis to Peritoneal or Home Hemodialysis.
Key Responsibilities- Partner with doctors, DaVita admissions, and home program managers to ensure a smooth intake process
- Perform administrative duties, including verifying patient insurance, answering incoming calls, scheduling appointments, and managing patient charts
- Facilitate outreach communication at all stages of the intake process, from admission to Day 1 with patients, in-center contacts, physicians, and home program managers
- Must be flexible and willing to travel
- High school diploma or equivalent
- Proficient in Microsoft Office, particularly Outlook and Excel
- Strong, professional written and verbal communication skills at all levels of the patient care team
- Highly organized and able to work well with deadlines
- Bilingual and Spanish preferred
