We are seeking a Senior Health Leader to join our team at Humber River Health. As a Nurse Practitioner, you will play a key role in delivering high-quality patient care and developing strategies to improve health outcomes for older adults.
About the JobThis is an exciting opportunity to work with a dynamic and inclusive team that is committed to providing exceptional patient care. As a Senior Health Leader, you will have the opportunity to develop and implement innovative solutions to improve health outcomes for older adults.
Key Responsibilities- Provide comprehensive health services to older adults, including assessment, diagnosis, treatment, and management of chronic stable conditions
- Develop and implement strategies to improve health outcomes for older adults, including education and training programs for staff
- Collaborate with physicians and other healthcare professionals to provide high-quality patient care
- Conduct research and quality improvement initiatives to advance health outcomes for older adults
- Master's degree in Nursing or related field
- Current registration as a Nurse Practitioner with the College of Nurses of Ontario
- Minimum 3 years of experience as a Nurse Practitioner in a geriatric setting
- Excellent communication and interpersonal skills
- Demonstrated ability to work independently and as part of a team
