About the RoleThe Long-term Care Coordinator is responsible for:
- Coordinating day-to-day operations and quality individual care within designated clinical areas
- Supporting the seamless movement of individuals through the healthcare system and accessing appropriate resources
- Supervising and serving as a role model and resource to staff for clinical decision-making, clinical assessments, and counseling and education of families
- Reviewing, analyzing, and making recommendations on administrative systems, such as policies, procedures, and standards of individual care, staffing levels, and quality improvement environments
- Liaising with the interdisciplinary team for resource identification, coordination, and efficient utilization, as well as planning for staff educational needs

- Graduation from an approved School of Nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM)
- Three years of recent related clinical experience in the assigned practice environment and two years of recent supervisory and teaching experience, or an equivalent combination of education, training, and experience
