About the Role

The Long-term Care Coordinator is responsible for:

  • Coordinating day-to-day operations and quality individual care within designated clinical areas
  • Supporting the seamless movement of individuals through the healthcare system and accessing appropriate resources
  • Supervising and serving as a role model and resource to staff for clinical decision-making, clinical assessments, and counseling and education of families
  • Reviewing, analyzing, and making recommendations on administrative systems, such as policies, procedures, and standards of individual care, staffing levels, and quality improvement environments
  • Liaising with the interdisciplinary team for resource identification, coordination, and efficient utilization, as well as planning for staff educational needs

Qualifications

To be successful in this role, you will require:

  • Graduation from an approved School of Nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM)
  • Three years of recent related clinical experience in the assigned practice environment and two years of recent supervisory and teaching experience, or an equivalent combination of education, training, and experience
